Early intervention saves lives and money. Through consistent preventive care, the clinic has helped manage over 25,000 cases of chronic conditions like diabetes, hypertension and heart disease - preventing costly complications that would otherwise burden our healthcare system and community resources by an estimated $75 million in long-term care costs.
